REGISTRATION FORM (For both Ultimate Tennis and Junior Training Camp)
MEDICAL FORM 3 (*Only necessary if your child requires medication while at
camp)
MEDICAL FORM 4 (*Only necessary if your child requires medication
while at camp)
To participate in any camp of our nature within
the state of Massachusetts, a camp physical form must be completed
by a physician. Most doctors have their own which we will
accept. If your doctor does not have one, please let us know and
we will send one to you.
